What to include

When you complete one of our referral forms, the below are must-haves – without them, we can't accept your referral. If you have your own referral form, please make sure you include this information.

  • Referrer details

Including your email address, GP surgery address and information about your ICB.

  • Patient details

Including NHS number, contact details, relevant referral documents - like clinical reports.

  • Risk information

Including any requests regarding escalating clinical priority and the rationale for this.

  • Reasonable adjustments

Including any changes we can make for the patient's needs and accessibility.

Important

Referring a child (aged 6-17)?

You must include the details of whoever has parental responsibility for the child or young person. This might be a parent, guardian or another adult.

You should also share any relevant details or evidence, whether observations from a teacher or SENCo, or from another adult who sees the child regularly, like a coach or tutor.

We'll request this as part of the pathway, but anything you already have helps us build a full picture more quickly.

Next steps

Once we've processed your patient’s referral, they’ll get a welcome email inviting them to our online portal. The process then looks like this:

  1. Pre-assessment forms

Your patient completes a set of forms

    • These are internationally recognised, and help us understand their physical and mental health, history and day-to-day life

    • Forms are completed by the patient, or in the case of a child, may be completed by a parent/guardian

    • For some adult assessments, we'll also need one completed by someone who knew them well as a child

  2. Assessment and titration

    We'll handle it from here

    • Once we have everything we need, we'll book in your patient's assessment

    • This is usually completed online, and we will make any reasonable adjustments necessary

  3. Patient outcome report

    We'll send this to you

    • Once they patient has completed their assessment and our clinicians have reached a diagnostic decision, we’ll share this with you in an outcome report

  4. Ongoing review

We'll continue to support your patient

    • In the case of ADHD medication, we'll provide your patient with regular reviews

    • Once stabilised on their medication, we may request a Shared Care Agreement with you.

    • It's up to your practice whether you accept the agreement

    • In the event you don't accept, we can continue to prescribe and monitor your patient privately

What if my patient is already awaiting an assessment with another provider?

You can make a new referral to us if the patient is waiting with another provider. Under the NHS Patient Choice framework, a patient can still exercise their Right to Choose even if they are already waiting with another provider.

Can I refuse to refer a patient to Noodle?

Yes, if you feel it is not clinically appropriate to do so. However, under NHS Right to Choose, if it is clinically appropriate, you must respect your patient's choice of provider. Our GP referral template letter explains their rights clearly if this comes up.

How will I know when a referral's been received?

If you sent the referral via e-RS, we'll add a comment to confirm that the referral has been processed. If you sent the referral by email, we'll send you an automated response to confirm it has been received.

Can a patient switch between NHS and private care?

In some circumstances, yes, but this depends on the stage of the referral and individual circumstances. NHS and private pathways cannot run concurrently, and there may be implications for future treatment and prescribing arrangements. For advice about a specific patient, please contact us on the emails below rather than relying on this guidance alone.

Adult enquiries: nhsreferralenquiries@noodlehealth.com

Under 18s enquiries: nhsreferralenquiries-cyp@noodlehealth.com

Do I need to provide a patient's Care Summary Record?

You only need to send one if it isn’t already included in the referral or accessible on EMIS. As long as it’s there and dated within the last six months, we won’t ask for it separately.
